ObjectiveTo study the effect of using improved 2D computer-assisted fluoroscopic navigation through simulating 3D vertebrae image to guide pedicle screw internal fixation.MethodsPosterior pedicle screw internal fixation,distraction and reduction were performed in 66 patients with thoracolumbar fractures and lumbar diseases.Anterior posterior,lateral and oblique X-ray images of vertebrae were obtained by improved computer-assisted fluoroscopic navigation,and subsequently anterior posterior,lateral spinal image and axial pedicle image were simulated to guide pedicle screw internal fixation.ResultsTwo hundred and eighty pedicle screws were implanted.Compared with real pedicle screw image,the mean position deviation of navigation virtual path was 1.5 mm and the mean angle deviation is 1.0°.Postoperative CT showed that all pedicle screws were situated within the pedicles without lateral or inner pedicle cortex invasion;the mean implant screw sagittal angle deviation was 2.0°compared to preoperative predicted results.ConclusionImproved 2D computer-assisted fluoroscopic navigation through simulating 3D vertebra image for pedicle screw internal fixation guidance is a simple,convenient and effective method which improves the accuracy and safety of pedicle screw implants.11 refs,1 fig.
